Permission slips for all trips will be located inside your learner’s PowerSchool Account. From the Navigation section window on the left hand side, select the “Forms” icon. Please note that each field trip will have a separate permission form.

Additional Paperwork

In order to dispense any over-the-counter or prescription medicines during a field trip, we must have your permission in advance. The following forms, if needed, should be completed each school year (or as needs arise/change).

Drivers

We can’t make these field trips happen without parents to drive/chaperone our learners. It’s a great way to get to know your learner(s)’ classmates, educator(s), and other parents. We’ll do our best to allocate trips across all interested parents (or grandparents, guardians, etc.). We’ll prioritize drivers who have completed the driver/volunteer forms in PowerSchool and other Level III volunteer requirements (LiveScan background check, TB screening). Signing up does not guarantee you’ll be selected, but we’ll try to confirm drivers (and backups) as far in advance as possible. When you sign up please include the # of spots you have for booster seats in your car in the “# of spots” field.

From the Navigation section window on the left hand side, select the “Forms” icon. Click on the Chaperone/Driver Volunteer Application Form.

  • The Driver Application requires the following uploads: driver's license, insurance declaration form (showing coverage levels), copy of drivers record ($2 download at Dmv.ca.gov).

  • Driving learners other than your own is considered a Level III volunteer role which also requires upload of the following: Proof of Covid vaccination, TB Risk Assessment signed by a Healthcare Provider (valid for four years).

All drivers must have LiveScan background check (completed once during your time at SCC). When completed, please email Charlene Nelson so she can keep an eye out for it once your fingerprints are cleared.

Voluntary Contributions

At SCC, we rely on voluntary contributions from our families in order to support our Field Trips programs. Funds not received from family donations are taken from the school’s general fund.

No learners will be excluded from any educational activity or program based upon their family’s inability or unwillingness to contribute. All voluntary contributions will be collected via CheddarUp. Payments can be made by e-check, credit/debit, check or cash. E-check has a notably lower transaction cost than credit/debit cards, so please consider paying by e-check. Specific questions about Field Trip donations can be addressed to Jessika Welcome, SCC’s Director of Business Services.

The suggested donation is $625/learner.
